Medication Review From Notes
Revolutionising Medication Reviews
Not all medication reviews require 15 minutes…
This was the thinking which led us to develop this service, which has quickly grown to become one of our most popular services!
We use evidence based criteria to risk stratify the patient cohort, taking co-morbidities, frailty and number of prescriptions into account, as well as ensuring the correct monitoring is in place and up to date - we are able to offer medication reviews as a purely administrative task.
As a result, our service essentially eliminates any chance for a DNA and requires no time usage on interpreter services.
The service is GP Led, meaning there is always oversight and GP clinical supervision provided and all clinical queries are dealt with in house by GP Specialists.

Transparent Pricing
PCN Funded
Fully Customisable Protocols
Our process with how we risk stratify and classify patients into different appointment slots is made available to the practice before the service begins, detailing exactly where the cost and time saving will be for the practice and their staff.
This service can be secured via PCN ARRS funding (or direct engagement). Our pricing is fixed per volume of work delivered - there are no fluctuations in output or hourly rates. This allows you to plan your funding better knowing exactly how much work will be processed.
We tailor our clinical guidance to fit your protocols, as each practice may have specific nuances and particular issues that need addressing.
Cost-Saving Service
By completing medication reviews on the practice’s behalf, 60 hours of staff time was freed up, which allowed staff to dedicate more attention to patient care and other critical tasks.
The reduced workload had a direct return on investment. By outsourcing medication reviews, the practice saved around £2,220, compared with the cost of completing these in-house. This comparative cost estimate was shared with the practice before any work was conducted.
All cases handled by us were vetted as suitable. There was a 0% DNA rate, and no interpreters were required.
All cases which may have raised a clinical query were dealt with in-house by our GP Led clinical team. This GP clinical supervision and support is provided to our staff when required 24/7 - this means that no work is ever passed back to the practice. This supervision is at no cost to the practice or PCN!
